Level 4 lockdown travel permit forms

You need a travel permit to travel to and from Gauteng, unless you are a resident of Gauteng who is returning to their place of residence.  The following forms apply:

If you have to move to or from Gauteng during the lockdown period (to a new place of residence)

Complete form 6 which is a sworn affidavit that has to be stamped at a police station or magistrate’s court.

If you have to travel to and from Gauteng for a     funeral

Complete form 4 which is a sworn affidavit hat has to be stamped at a police station or a magistrate’s court.

Also note there is a restriction of 50 people at any     funeral.

If you have to travel to and from Gauteng to perform a service (for work / business)

Complete form 7, which is a declaration by the owner or the head of the institution or business, signed by him and stamped with the company stamp.  The person to whom the permit is issued, must carry it and their ID at all times during travelling to and from Gauteng and   during the time spent there.  It may be required for    accommodation reservations as well.
